Who is Eligible for NDIS Disability and Aged Care?

To be eligible for these services in Australia, you must satisfy certain age, health and other criteria.
 

There are two primary government-funded programs designed to help you stay in your home longer. Your eligibility depends on your age, health requirements, and personal circumstances.

Funding for Aged Care

You must meet specific requirements to be eligible for an assessment. Among them are:

  • The degree of support required for daily duties
  • Being 65 years of age or older (for Aboriginal or Torres Strait Islander people, 50+)

Note: If you are 50 or older, have a low income, are homeless, or are in danger of becoming homeless, please visit myagedcare.gov.au or call the My Aged Care contact centre at 1800 200 422 to discuss your circumstances.

Funding for Disability Care

You must meet the following requirements to be eligible for an NDIS assessment:

  • The disability must significantly affect day-to-day functioning because of permanent and significant disability.
  • You must be between 7 and 65 years old.
  • Citizenship, permanent residency, or possession of a Protected Special Category Visa are prerequisites for eligibility in Australia.

Note: The disability must impact the person’s capacity to engage in social and economic activities.

Government Funding

Federal and state-funded programmes are available to support your independence and overall well-being. Each programme has its own requirements, and your location may affect your access to certain programmes. CareLinc can assist at every stage of the application process.

The Pathway to Support:
My Aged Care

Government-funded support starts with a registration through My Aged Care. Here is how CareLinc can guide you through the process:

Step 1

Registration & Referral

Call My Aged Care at 1300 150 372 or apply online. You can also ask your GP or a health professional to refer you directly.

Step 2

Assessment

An assessor will meet with you to discuss your health, goals, and daily needs. They use the Integrated Assessment Tool (IAT) to determine what level of help will best support your independence.

Step 3

Approval & Support Plan

You will receive a Support Plan and a “Notice of Decision.” This confirms your eligibility and the level of funding approved for your care.

  • A support plan outlining agreed goals and recommended services, and
  • A Notice of Decision, confirming your eligibility and approved support level.

If approved for funded services, you’ll receive a referral code to share with your chosen provider.

Step 4

Choosing CareLinc

Once you have your referral code, contact us. We will work with you to turn your approved plan into a personalized care schedule that respects your choices.

Home Care Package (HCP)

This is an Australian government-funded aged care program helping people live independently at home. The Support at Home Program will replace HCP from 1st November 2025.

 

Commonwealth Home Support Programme (CHSP)

If you are already living independently in your home but need basic support, you might consider this programme, funded by the Australian Commonwealth Government.
 

National Disability Insurance Scheme (NDIS)

If you are living with a permanent or significant disability,  consider the NDIS, which is funded by the  Commonwealth, State and Territory governments. This scheme is not means-tested.

You can always pay directly for your home care services if your situation allows it. If private care is your preference, you can:

  • Select the home care services you desire rather than just those covered.
  • Receive in-home care while awaiting approval of your government financing.

If you only need minor assistance such as help with cleaning, transport, or meal preparation you may be eligible for the Commonwealth Home Support Programme (CHSP). This program provides basic, flexible help as you transition toward more comprehensive care.

The New "Support at Home" Program (Commencing July 1, 2025)

The Australian government is reforming aged care to make it simpler and more transparent. The new Support at Home program will replace current models to provide better quality care and more choice for older Australians. At CareLinc, we are fully prepared for these reforms and will ensure your transition to the new system is seamless.

A New Standard of Support for Older Australians

Australia’s aged care system is undergoing its most significant transformation in decades. These reforms are designed to ensure that the support you receive is not only high-quality but also easy to access and tailored to your specific needs.
